Laboratory Test Results of Cholera Outbreak Strain in Haiti Announced

　　−−−The rapid identification of the outbreak strain as Vibrio cholerae serogroup O1, serotype Ogawa and antimicrobial susceptibility profiles were reported last week by the NPHL. The new findings from CDC's laboratory are based on a method of "DNA fingerprinting" called pulsed field gel electrophoresis (PFGE), which analyzes DNA patterns that can then be compared with PFGE patterns of cholera strains from other regions of the world. The PFGE testing was performed on 13 bacterial isolates recovered from patients with cholera in Haiti. The PFGE analysis shows that these isolates are identical, indicating that they are the same strain and similar to a cholera strain found in South Asia.　（以下略）


で、コレラ菌についてあまり専門的に掘り下げずに調べてみますと；

  • −−−コレラ菌の学名はVibrio cholerae である。分類学的にV.cholerae は菌体表面のO 抗原（リポ多糖体）の違いによって、現在205 種類（11種類は未発表）に分類されている。このうち、コレラを起こすのはO1 およびO139 血清型のみである。コレラの典型的な臨床症状を起こすのはコレラ毒素であることがわかっているので、厳密にいうと、コレラの原因菌はコレラ毒素を産生するV.cholerae O1 およびO139 である。　（以上、 国立感染症研究所 感染症の話 コレラ より抜粋）
  • −−−O1型については、さらに吸収抗血清によって小川型(Ogawa, AB型)、稲葉型(Inaba, AC型)、彦島型(Hikojima, ABC型)という亜型に分類されている。また生物学的特徴の違いから、古典型（アジア型）とエルトール型という2つの生物型(biovar)に分類されている。　（中略）　コレラ菌のうち、コレラの原因になるものはすべてコレラ毒素（コレラトキシン、コレラエンテロトキシン）と呼ばれる毒素を産生する。O1コレラ菌の大部分と、O139コレラ菌の一部がこれに該当し、これらの菌がヒトの腸管内で作り出すコレラ毒素が、直接の病原因子として腸管に作用し（腸管毒、エンテロトキシン）、下痢や脱水症状などコレラ特有の症状を引き起こす。　（以上、 コレラ菌 - Wikipedia より抜粋）
  • −−−O1抗原をもつ菌をコレラ菌といい，O2 以下の抗原をもつ菌は NAG ビブリオまたは非 O1ビブリオとよぶ。なお1992年にベンガル地方で分離された O139 型ビブリオはコレラ毒素を産生し，コレラと同様の臨床症状を引き起こすため O139 ベンガル型コレラ菌とよぶ。　（中略）　抗 O1 血清を用いて O 抗原が１型であることを確認する。東南アジアのみならず，ヨーロッパ，アフリカ，アメリカ大陸で大流行が認められている。　（以上、 国際感染症臨床情報 より抜粋）

　　つまり 『今回のコレラ菌は南アジア地域のものと類似している』 と云う速報レベルの発表から短絡的にネパ−ル （部隊が川に捨てたとされる排泄物） が感染源扱いされている可能性大。ネパール部隊を警察が暴徒から守ると云う本末転倒な対応ではなく、容疑者の検査結果を公表して潔白を証明するべきと思いますが。上掲ＢＢＣの記事中 "The UN says it has found no evidence to justify the accusation against the Nepalese troops, but the cholera strain matches a South Asian one. The Nepalese army said tests had proved that the allegation regarding its personnel was false." とありますが、全く信用の無い国連お手盛りの発表では誰も信用しませんって。多分ハイチではその存在を求められていない国連ハイチ安定化派遣団などどうでもよいが、現地で奮闘されているアジア系の方々まで迷惑を蒙りかねない。

◆Haiti: report assails cash for work programs

−−−The report found that while much of the work is useful and the payments do provide temporary relief for the workers and their families, CFW programs fail to stimulate the Haitian economic significantly:　（中略）　a large part of the CFW money ends up going to other countries instead of producing permanent employment inside Haiti, according to Haitian economists. The programs also increase Haiti's dependency on other countries, deform attitudes towards work and weaken grassroots and communities efforts for reconstruction, the report said. The authors suggested that groups like the US Agency for International Development (USAID) are promoting CFW as a way to prevent the sort of militant demonstrations that thousands of displaced Mexico City residents organized to demand housing after a major earthquake there in 1985.
